Changing name on passport
The process can vary when it comes to changing the name on your passport. Below is a list of some of those situations and how to handle it.

- Your original or certified name change document, such as a marriage certificate or court order (no photocopies or notarized copies)
- Your valid passport

If you are eligible to use Form DS-82*, submit the following by mail:
- Form DS-82
- Your original or certified name change document, such as a marriage certificate or court order (no photocopies or notarized copies)
- Your valid passport
- One color passport photo
- Adult Renewal Applicant fees or Child Applicant fees, depending on your age
If you are not eligible to use Form DS-82, submit the following in person at an acceptance facility or a passport agency (restrictions apply):
- Form DS-11
- Valid ID and photocopy of that ID
- Your evidence of U.S. citizenship and photocopy of that evidence
- One color passport photo
- Your original or certified name change document, such as a marriage certificate or court order (no photocopies or notarized copies)
- Adult First-Time Applicant fees or Child Applicant fees, depending on your age
Please note: You do not need to submit proof of the name change if your name has changed due to marriage and you present an ID issued in your new name. You must include the details of the marriage in the appropriate section on Form DS-11.

You are already using a different name but cannot document the change with a court order, divorce decree, or marriage certificate |
Submit the following in person at an acceptance facility or a passport agency (restrictions apply):
- Form DS-11
- Your evidence of U.S. citizenship
- Valid ID issued in the name you are currently using and photocopy of that ID
- One color passport photo
- Adult First-Time Applicant fees or Child Applicant fees, depending on your age
Please note: If your ID is less than 5 years old, they may ask you for additional identification as evidence that you have been using the name for at least 5 years.

*You will be able to renew your passport by mail using Form DS-82 as long as your most recent passport is submitted along with the application, is undamaged minus normal wear and tear, was issued when you were 16 or older, was issued within the past 15 years, or was issued in your current name (or you can provide documents showing the name change with an original or certified copy of your marriage certificate or court order).
Correct passport information or printing error
If you have a passport that was issued with incorrect information such as name, sex, place of birth, or has a printing error, data is missing on biographical page, crooked printing, discoloration etc, then it can be corrected at no charge as long as the passport is still valid. In order to correct the information or printing error you will need to fill out
Form DS-5504. Once you have filled it out you will then need to submit the form along with your passport, one color photo and your evidence of the error. You do not need to submit any fees, unless you are requesting the passport expedited service. The validity of the new and correct passport will depend on when you report the error.

If you have a passport and you changed addresses you do not need to request your passport updated. However if you have a passport application that is currently being processed and you need to change the mailing address, please call them at 877-487-2778 or at 888-874-7793 for TDD/TTY.
If you believe your passport photo looks bad due to printing error on their part, you will need to fill out Form DS-5504 to re-apply for a new photo at no cost to you. However if your photo is printed correctly and you just don't like the current photo, you then will need to fill out Form DS-82 to re-apply, if eligible. You will then need to pay normal fees.
